Here at Niceville Family Dental Center we know that many of our patients have dental insurance to help supplement the cost of dental care. As a courtesy to our patients, we will file your primary insurance, provided they will assign benefits directly to us.  We ask that you bring a copy of your dental policy book, insurance card and a completed and signed dental claim form at your initial visit.  Without this information, we will be unable to estimate your benefits and you will be expected to pay in full at the time of your visit.  With insurance eligibility confirmed, we require payment of any deductibles and estimated co-payments when treatment is rendered.  Any balances not covered by insurance are the patient's responsibility.  Insurance assistance is available for policies that may not directly assign benefits to us. We will not, however, let insurance companies dictate the quality of treatment we recommend to be in the best interest of our patients.
